Forms and Policies
In order for your camper to attend the Burgundy Farm Summer Day Camp you will need to make sure you have the following forms (these are hardcopy forms that you need to submit in addition to all information that was entered online during registration!) on file with the camp administration.
*Please note: these forms are in addition to any and all information you entered during the online registration process. These are hardcopy forms that we need to have in hand so that they can be placed in the campers file. This is required by both our licensing and accreditation, and must be completed and turned in prior to the campers first day of camp. Burgundy will not be able to accept any camper who has not completed this requirement.
- VA Health Form (HIF Form)
- Proof of Identity Form (Child Registration Form)
- Medicine Authorization Form (only if medicine is to be administered at camp, including Epi-pens)

Reminder Regarding Cancellation and Refund Policy
All payment for the 2019 Burgundy Farm Summer Day Camp is due on May 15, 2019. Up until the payment deadline of May 15 you are able to make changes to your registration with no penalty (other than the non-refundable deposit). If you want to add more programs or cancel one, you will need to contact Hugh Squire or Laura Motaweh at 703-842-0477 and they will assist you.
After May 15, there are no refunds for any reason. If you add any camps or programs after May 15 payment is due in full at the time of registration.
