Camper Forms – What You Need to Know

To be fully prepared for camp, all families must complete the online registration AND submit the required camper forms listed below. These forms are hardcopy forms and are required in addition to the information entered online.


Required Forms for ALL Campers

Virginia Child Registration Form

This form is required by the state of Virginia for all campers.

  • Returning campers:
    Please complete page one with updated information and sign and date page two. Be sure to include the current camp dates (you may list the full summer: June 15–August 7, 2026).
    You do not need to complete the Camper ID section again.

  • New campers (or campers who did not submit this form for Summer 2025):
    Please complete the form in its entirety and arrange for camp staff to view the form in person along with an original Camper ID document (birth certificate, valid passport, etc.).
    Families may complete this requirement during Paperwork Playdates or by scheduling an appointment with the Camp Office.


Virginia Health Form 

This form is required to ensure we have up-to-date health information for your camper.

  • The form is valid for two years from the date of the physical exam (example: May 14, 2024 – May 14, 2026).

  • If you already have a current VA Health Form that will still be valid during Summer 2026, you may submit a copy.

  • The form includes three sections:

    • Parent-completed section

    • Pediatrician-completed section (wellness visit)

    • Copy of immunization records

  • We can accept health forms from other states or districts as long as they are current and complete.


Optional Forms (Only If Applicable)

Medication Forms

These forms are required only if your camper will need medication administered at camp.

  • 10 days or less (Over-the-Counter only)

  • 10 days or more (Prescription and/or Over-the-Counter)
    This form includes a section that must be completed by the pediatrician.

Important medication guidelines:

  • Burgundy requires our own Medication Authorization forms; we cannot accept forms from other schools or camps.

  • Forms must include detailed and accurate information, including:

    • Medication name (generic vs. brand)

    • Symptoms requiring administration

    • Dosage and method of administration

    • Dates the authorization is valid

  • Medications must be:

    • In original packaging

    • Not expired

  • Medications and forms should be placed together in a labeled ziplock bag with the camper’s name.


Please remember:

Submitting these forms is required in addition to online registration. A camper is not fully cleared for camp until both online registration and all applicable hardcopy forms are complete and submitted.

Submitting Camp Forms

  • In late spring Burgundy will host two Paperwork Playdate events that will allow families to come to campus and take care of their camper’s paperwork requirements as well as explore the campus and meet some of the camp staff.  Families who attend these events will want to be sure to bring EVERYTHING they need to satisfy their campers paperwork requirements (the actual hardcopy forms and valid ID’s).
  • You can also email your campers forms directly to the camp team.  Just email them to Hugh Squire (hughs@burgundyfarm.org) or Stephanie Salinas (stephanies@burgundyfarm.org). 
  • Remember, if you need to do the proof of ID for the VA Child Registration Form (new camper), that MUST be done in person.  You can come to one of our Paperwork Playdates in the late spring or contact the camp team to make arrangements to come in and satisfy this requirement.
